Cost of neglect

This is not “you might feel tired.” Chronic gaps in iron, vitamin D, B12, calcium, protein and fibre show up as measurable tissue failure - slower myelination and a 5-20 point IQ decrement in severe early iron deficiency, osteoporotic fracture after the peak-bone-mass window closes, sarcopenia that turns a fall into a hospital stay, neuropathy that gets filed as aging, and cardiometabolic disease that is the largest diet-linked bill a family will ever pay. The cost is clinical, then functional, then economic: higher lifetime medical bills, fewer productive years, and in children a quieter form of lost potential.

A. Habits that damage when they dominate

Ultra-processed snacks as a food group

Packaged namkeen, instant noodles, and biscuits are salt, refined starch, and oils at snack density. They displace dals, curd, fruit and nuts - the potassium, fibre, protein and magnesium this atlas keeps asking for.

Sugar-sweetened drinks

A 300 ml cola is not a potassium food. Fructose-and-glucose boluses feed hepatic fat (see Liver) and glomerular stress (see Kidneys). Coconut water and chaas are the summer replacements that actually contain potassium.

Excess refined flour, daily

Maida strips the magnesium and fibre the wheat berry had. A paratha is a food; three maida parathas plus no dal is a glucose and magnesium problem.

Trans-fat fried food, reused oil

The evening pakora from oil that has been browned twice is how LDL oxidation gets a kitchen source. Winter and monsoon stalls are the usual scene.

Alcohol in excess

Hepatocyte fat, emptied folate, emptied magnesium, emptied zinc, emptied testosterone, emptied sleep. There is no organ on the map that bills this as a tonic.

Skipping protein at breakfast

The leucine trigger is per meal. A biscuit-and-chai morning means the myocyte, the follicle, and the school-attention system wait until 2 pm. Older adults cannot catch up at dinner.

Tea with iron-rich meals

Tannins bind non-heme iron. A dal-palak lunch with chai on it is how a vegetarian 18 mg target becomes a 4 mg absorption. Move the chai an hour away.

B. Nutrients people skip, and the downstream cost

Iron in early childhood

7 months to 3 years, especially if complementary food is only cereal and milk.

Cognitive lag that follows the child into school and work. Severe early deficiency: 5-20 IQ points, only partly reversible. This is not a pale face. This is a quieter form of lost potential.

Vitamin D + calcium across adult life

Teens who skip dairy and sun; women after 50; anyone indoor and covered.

Missed peak bone mass, then fracture, then loss of independence, then a nursing-home trajectory. A hip fracture is among the most expensive diet-related events in later life.

Fibre and plant diversity

Anyone whose plate is maida, meat or cheese, and a pickle.

Higher cardiometabolic disease - the largest diet-linked economic cost globally. CVD and diabetes dominate the bill. Soluble fibre from oats, barley and dals is the LDL mechanism, not a wellness slogan.

Protein in older adults

51+ still eating the 0.8 g/kg RDA, biscuit breakfast, dal only at night.

Sarcopenia, falls, longer hospital recovery, loss of independence. Muscle is the organ that gets you home from the ward.

B12 in older adults or vegans

No animal food and no tablet; or over 50 with low acid, metformin, PPIs.

Neuropathy and cognitive fog that can be mistaken for ‘just aging’, then a walker, then a label of dementia that a cheap vitamin might have prevented if caught early.

Iodine in pregnancy

Rock-salt kitchens, no fish, no dairy, no iodised salt.

Fetal brain injury at the extreme; quieter IQ cost at milder deficiency. Iodised salt is the intervention that already exists.

Choline on egg-free vegetarian plates

No eggs, no soy, no liver - a common urban vegetarian pattern.

The liver cannot export fat (MASLD even in non-drinkers) and the fetal brain undershoots in pregnancy. Two eggs is a clinical food, not a breakfast optional.
